Registering and Admissions
All outpatient and inpatient registrations for appointments, procedures, and appointments with Dr. Tom Kerr will need to register at Cogdell Hospital!
Admission
The Admitting department is located in the front lobby for Outpatient
and Inpatient Registrations. The hours of operation are Monday through
Friday from 7:00 a.m. to 6:00 p.m.
The ER registration entrance is on
the North side of the Lobby entrance. It is staffed 24 hours a day and 7
days a week.
The Admission process consists of signing forms, verifying your
identity, and obtaining copies of your insurance cards. Please be aware
that at this time, we verify all insurance eligibility and provide
information of your co-pay and deductible responsibilities. Although we
strive to complete this process quickly, it may take several minutes.
We
request that you plan your registration time accordingly to your
scheduled visit.
Patients who are not insured will be requested to make a deposit on
their account prior to or at the time of admission. A discount will be
given if the amount is paid in full at time of admission.
Be prepared to present:
• Current photo ID
• Any insurance cards
• Worker’s Compensation Information
• Other eligibility forms
• Doctor’s orders
• Advance Directive
• Other information pertinent to your registration
Please note, Any tests or procedures performed that require reading
or review by Cardiologists, Pathologists, Radiologists, etc. will be
billed separately by those entities, apart from any invoicing by Cogdell
Hospital.
ID Bands
A patient identification wristband will be applied during
registration to ER, Inpatient, OB and OBT patients only. This ID band is to ensure that proper tests and
medications are dispensed correctly. When you are ready to leave the
facility, make sure you or a staff member disposes of the ID band
properly.
Pre-Registration for Scheduled Admissions
For your convenience, a pre-admission process is designed to make your
admission to the facility as simple and efficient as possible.
You may
be notified by phone to obtain correct insurance, demographic and
guarantor information. Also, you will be advised of your co-pays and
deductibles. Upon arrival to the facility, check in at the front
Admitting desk for payment, signatures, wrist band, if applicable, and to pick up your
paperwork for your scheduled appointment.
You can also Pre-Register in the Admitting Department at your
convenience. Again, remember to have all necessary information available
for an effective registration.
Charity Care Program
Cogdell Memorial Hospital does have the Indigent Health Care Program
which meets the state and federal guidelines for a hospital district.
This program provides unreimbursed services to financially and medically
indigent patients who qualify, regardless to race, creed, color, or
national origin. Other programs may be available.
