Going Home
Discharge Instructions
It's likely your doctor will provide you with information about follow-up care and instructions before you leave. If you have questions about caring for your condition or treatment at home, talk with your nurse.
Checking Out
When your doctor has discharged you and you are ready to leave, you will be escorted out in a wheelchair by hospital staff. Please make sure you have family or friends with you to give you a ride home!
Before leaving, be sure to check your room carefully for any personal belongings.
Home Health Services
If you feel you may require assistance at home following discharge, ask your doctor or nurse about home health services. There are several Home Health Providers in our area and you are free to select the one you want.Traditional home health care services include skilled nursing, physical therapy, occupational therapy, respiratory therapy, speech therapy, social services, laboratory services and home health aides.
